Power of Attorney Resignation Form (Ontario)

This is a form that a person acting as attorney under a Power of Attorney for Personal Care or Continuing Power of Attorney for Property can use if they wish to resign in accordance with Ontario law. It contains a fill-in-the-blank resignation form, information about who gets a copy, and a copy of the relevant section of the law.
